Passports Lost or Damaged in the recent disasters
Instructions and forms are available online at https://travel.state.gov/disaster. Applicants will mail in their documents themselves and there is no fee unless you require expedited processing.
For urgent replacement (travel in 2 weeks or less), you will need to make an appointment at the main passport office located in the Wilshire Federal Building. They can make an appointment online at https://passportappointment.travel.state.gov/.
For regular passports -
Appointments are required, we are unable to take walk-ins.
To see our current hours and make a passport appointment with the Thousand Oaks Library, please visit our Passport Services page. Unfortunately, we cannot make your appointment for you either in person or over the phone. Not seeing an available appointment after several tries is not a sign that the system is down. During the Spring and Summer especially, it may be quite challenging to find appointments.
We process new passport applications - form DS-11 only. We do not process renewal forms DS-82, second passport forms, or provide other passport services.
Paper copies of the DS-11 and DS-82 forms are not currently available as the U.S. Department of State is currently updating them. All persons are instructed to use the fillable online forms until the new versions are available.
Applications, including all of your supporting documents will be mailed to the National Passport Processing Center. The documents will be returned to you by mail.

Here is a Checklist of everything you should bring to your appointment. Remember to make photocopies of your documents ahead of your appointment time.
Note: It can be quite hard to get an appointment, if you are unable to get one through our site, you may want to check for Post Offices that process passport applications at https://tools.usps.com/rcas.htm. Not seeing an available appointment after several tries is not a sign that the system is down. During the Spring and Summer especially, it may be quite challenging to find appointments. When the USPS appointment system seems to be giving an odd response, try scrolling up, you will often see at the very top that the location selected shows no available appointments in red letters.
Most passport locations do not have walk-ins at this time, one exception we know of is the Van Nuys Post Office Passport Mega Center - they do have walk-ins allowed Monday - Friday from 9:30-6 and Saturdays from 9-5. If you choose this option, please be aware that they caution users that being in line to walk-in does not guarantee that a person will be able to see an agent and process their application. The phone number to Van Nuys (818) 374-5650 accesses an information menu only, no appointments or passport advice are offered by telephone. You may see online sites that say Oxnard Post Office or Pt. Dume Post Office allows walk-ins, this is old (pre-pandemic) information. As of 02/04/2025, Oxnard and Pt. Dume have appointment only services for passports.
