What do I do if I lost my library card?

How can I replace my lost or stolen library card?

I lost my card.

I think I lost my card.

I don't have my library card with me, but I know I still have it. 

I signed up online, but I forgot to make a note of my card number.

If you have lost your card, please come in to the Thousand Oaks or Newbury Park Library and show current Government issued ID to get a new card.  There is a $2.00 replacement fee for a new card. 

If you misplaced your card, but think you can find it, you can check out items at the libraries by showing current Government issued ID at the checkout desk. 

Can't find your card number:  Your library card number is printed on your library card, if you sign up for a library card online, you will see the card number appear on the screen after you sign up.  If you can't find your card number and you are using a Virtual card and are unable to come in to the Library in person, please let us know, at Ask-a-Librarian, we can delete your account so you can sign up for a new one. 

We are very sorry, but due to privacy concerns, we cannot provide library card numbers online or over the phone.
